FACILITY RENTALS

By signing this waiver I agree to all the information included therein for myself, my group and any person that is a guest of mine. It is my responsibility to ensure all guests are aware of all items included in this document, lack of awareness does not absolve any party from being held to the terms below. I hereby state that I recognize the term "Rage Activity Center" to be the building used by the organization Southern Tier Rage as it relates to this waiver. I further recognize this waiver covers the organization, its coaches, staff and volunteers. Southern Tier Rage is a non-profit youth basketball organization that partners with PHD and personal information will be shared between the two organizations only and only as needed. Anthony Lindsey and all volunteers and staff acting on behalf of Rage are not liable for stolen/damaged property, injury or death that may occur. While in our facility you and all those you bring in MUST follow all of our rules, including but not limited to: • NO Outside shoes on the court. Spectators need to walk on floor mats and sit only on bleachers or team bench areas. • Other than bottled drinks or those in reusable bottles, NO outside food, drink or coolers are allowed in the building. • Any garbage must be disposed of properly before exiting. • Members of your group MUST stay only in permitted areas. • Concession sales are NOT allowed unless an agreement is reached in writing to do so between the group and Tony Lindsey. All participation is voluntarily whether it be as a participant, coach, official or spectator and comes with assumed risk of said participation. This risk is not confined to just inside the gym but extends to common areas and parking lots/driveways. I understand and accept these risks and agree that I am responsible for communicating potential risks to any guests I bring or invite to the program. All staff within our organization are background checked and have agreed to proper procedures for interaction with minors. Groups reserving the facility SHOULD do the same. The group using the facility is responsible for ensuring proper supervision of their members to ensure only appropriate adults are interacting with the children. In addition, I hereby state that if I, or anyone I sign on behalf of, contracts any infectious disease, sustains any injury or has lost/stolen property through participation in any form, I will not hold Southern Tier Rage, the owner of the building housing the Rage Activity Center, Anthony Lindsey or any Rage volunteers/staff responsible. I realize there are risks and I accept them as my responsibility, whether expected or otherwise. We are responsible for replacing to the satisfaction of Tony Lindsey or financially compensating Southern Tier Rage for any damages to facility or equipment caused by our group.
